When life changes, your plan can change too. Whether you’re finishing school, moving house, leaving Auckland or experiencing health changes, we’re always here for you. Helping you get the support you need to live your best life, today and tomorrow.

Reviews and reassessments

Your reviews and reassessments are scheduled in advance, so you don’t need to worry about calling us to set them up. Here’s how it works:

  • Reviews are scheduled every 12–24 months. If needed, they can be brought forward to 3 or 6 months.
  • Reassessments typically happen every 3–5 years.  These happen less frequently if your needs have remained consistent and stable.

If your situation changes before your scheduled review or reassessment, you can contact us anytime.

Important times to contact us

We’re here to support you through life’s challenges, and one of the best ways to do that is by planning ahead. So get in touch if you’re expecting a big change, or if you experience any of the following:

  • If you think the support you’re gettng from us need to change because you’re starting or leaving school
  • You’re moving out of (or back to) Auckland
  • Changes in who provides your support
  • Concerns with the person or provider supporting you
  • Your living situation changes
  • You needi more help supporting your disabled loved one or taking care of your own wellbeing
  • If you’d like us to work with other agencies supporting you
  • Periods of hardship or struggle.

Some aspects of your support may change over time for a variety of reasons — and this is completely normal. If any adjustments happen, we’ll work closely with you to navigate these changes and explore additional supports from other agencies that can help meet your needs.
