It’s really important that we have your most up-to-date contact details. Here’s why:
- To keep you informed about your supports and our services.
- You don’t miss important updates or reminders.
- So we can reach you quickly if something urgent comes up.
Here’s another important reason: we need to be able to contact you when it’s time for your reassessment, which happens every five years. During a reassessment, we talk with you about whether your supports are still working for you. If we can’t reach you, your supports may not continue as expected, because the Ministry requires us to confirm them with you.
We will always do what we can to contact you before that happens. This includes reaching out to your preferred contact person, your emergency contact, your other providers or GP and other contact details we may have since the last time we spoke.
