When making a referral…

  • Have any supporting documentation ready before you start.

  • You can always save your online referral and come back to it later.

  • Give the disabled person’s full name.

  • If making a referral on behalf of someone, include your name (and make sure you have their consent).

  • Give a current contact number and email.

  • Attach a clinical letter (a clear photo of the letter will do) from the GP or specialist, describing the disability and how it affects you and your family. You can also ask your GP to send a referral on your behalf.
