How Needs-Based Support Works

The disability supports landscape can be challenging to navigate. We help disabled people, families and whānau find their way through the maze. Guiding you towards the right funding, information and resources for you.

1. Make a referral

You can make a referral for yourself, or someone can make one on your behalf. The process is easy, and you can do it all online. Make sure to include any clinical reports explaining your disability, how it affects your life, and what your needs are. This will help to move your referral forward, and get you answers faster about whether you can get support.

2. Let’s talk

You might’ve heard of a ‘needs assessment’ or NASC. But we like to think of it as a kōrero. So let’s talk. You can tell us as much as you’re comfortable sharing. We’ll take the time to listen, learn, and discover how we can help you live your best life.

3. Create your supports

We’ll use the information you’ve shared with us to put together our disability support recommendations. We’ll explain how these supports work, the providers who can help put them in place, and talk through any and every question you might have.

4. Use your supports

How you use your plan will look different, depending on the types of supports it involves. Whether it’s Flexible Support, Contracted Support, or a bit of both, we’ll be right by your side. Helping you navigate your responsibilities, manage your plan safely, and make sure it’s helping you live your best life.

5. Change your supports

Life is full of changes, and if things have shifted for you, we want to hear about it. Talk to us, and we can make any changes needed to your disability outcomes plan. So we can be sure it evolves with you and your life.
